초기 성인의 아동기 부정적 경험과 자살사고: 우울의 매개효과와 사회생태학적 레질리언스의 조절된 매개효과

초기 성인의 자살 문제는 사전 예방과 조기 발견의 중요성이 강조되고 있어 예방적 관점에서 자살사고에 미치는 결정적 요인과 효과적인 개입방안을 모색하는 것이 무엇보다 중요하다. 지금까지 자살사고에 관한 실증연구는 스트레스, 불안, 우울 등 정신건강 문제의 치료적 접근에 관심이 있었다. 그러나 자살은 어느 한 시점에서 결정되는 행동이 아니라 전 생애과정에 영향을 미치기 때문에 아동기의 심리적 외상의 논의로 확장될 필요성이 있다. 또한, 아동기 심리적 외상이 우울, 자살사고에 미치는 과정에서 사회생태학적 레질리언스의 영향력을 살펴봄으로써 개인, 가족, 지역사회 개입수준의 사회복지적 실천방안을 모색되어야 한다. 이에 본 연구에서는 아동기 부정적 경험(Adverse Childhood Experiences)이 자살사고에 미치는 영향력과 그 관계에서의 복합적 요인인 우울의 매개효과를 검증하였다. 또한, 사회생태학적 레질리언스의 조절된 매개효과를 검증함으로써 사회복지실천적 개입전략을 모색하고자 하였다. 이를 위해 만 18세~28세 남녀 총 309명을 대상으로 조사하였고, SPSS 16.0과 Hayes의 Process macro v. 3.6 for SPSS를 활용하여 분석하였다. 

 주요 분석 결과는 다음과 같다. 첫째, 초기 성인의 아동기 부정적 경험과 자살사고 간 유의미한 정적 상관관계가 나타났으며, 그 관계에서 우울은 부분매개효과를 나타냈다. 둘째, 초기 성인의 아동기 부정적 경험이 우울을 매개로 자살사고에 미치는 영향에서 사회생태학적 레질리언스는 조절된 매개효과가 검증되었다. 또한, 사회생태학적 레질리언스 하위요인을 분석한 결과, 개인 레질리언스는 유의미하지 않았지만, 관계 레질리언스와 맥락 레질리언스는 유의미한 조절된 매개효과를 나타냈다.

 주요 연구 결과에 대한 함의와 제언은 다음과 같다. 첫째, 초기 성인의 아동기 부정적 경험은 직접적으로 자살사고에 영향을 미치는 동시에 우울을 매개로 자살사고에 이르는 결정적 요인으로 작용하므로 초기 자살사고 예방과 개입을 위해서는 우울에 관한 치료적 접근과 더불어 아동기 부정적 경험에 대한 개입의 논의로 확장되어야 한다는 점을 시사한다. 그러므로 사회복지실천 현장에서 초기 성인의 자살사고 개입에 있어 아동기 부정적 경험 이해 기반 사회복지실천(ACEs-informed social work practice)적 개입전략이 논의되어져야 한다. 둘째, 초기 성인의 자살사고 개입을 위해서는 사회생태학적 레질리언스의 실천적 관점과 실천 개입전략이 유용하다는 함의를 가진다. 특히 하위요인 분석에서 관계 레질리언스와 맥락적 레질리언스가 유의한 조절된 매개효과가 검증되었으므로 자살사고 예방을 위해서는 전 생애관점에서 가족 및 사회적 자원 강화를 위한 실천적 개입과 사회 시스템이 구축되어야 한다. 

 본 연구의 의의는 초기 성인의 자살사고 개입을 위한 사회복지 실천적 관점과 모델을 제시하고, 사회복지 실천계획과 전략을 마련하는데 주요한 기초자료를 제공하는 데 있다. 


Suicide is the leading cause of death in early adulthood among 20-year-olds, such a social problem requires active intervention. To date, most studies on suicidality and related factors have focused on therapeutic approaches to mental health problems such as depression. However, as suicide is not a behavior that is not a behavior determined at a certain point in time but one which affects the entire life cycle, it is necessary to expand the discussion of interventions for psychological trauma from a life process perspective. Many previous studies have shown that Adverse Childhood Experiences(ACEs) in early childhood have robust associations with suicidal ideation in adulthood. These effects can be cumulative, with a greater number of ACEs progressively associated with suicidal ideation. Thus, to understand suicidality in early adulthood, it is necessary to confirm the association between ACEs, suicidal ideation, and depression. Furthermore, through the application of social ecological resilience, an attempt was made to propose a social work strategy for the prevention of suicidal ideation. 

 The purpose of this research was to determine whether depression plays a mediating role between ACEs and suicidal ideation in the effect relationships and to examine whether social ecological resilience had moderated mediating effects. 

 The study model was designed based on SPSS 16.0 and PROCESS v.3.5 for SPSS by Hayes. A total of 309 samples were selected for the study, which consisted of male and female adults aged 18 to 28 years old, then utilized data collected through a survey for the analysis. IRB approval was obtained for ethical review of the present study.

 The main findings of this study are as follows:

 First, the ACE score had a strong, graded relationship with suicidal ideation. Compared with those without an ACE, those with at least one ACE were significantly more likely to report suicidal ideation. These findings indicate that for prevention and intervention of suicidal ideation, the discussion of interventions on ACEs should be broadened from a life process perspective. In other words, suicidality intervention should address ACEs and, most importantly, seek policy and practice interventions for prevention. In addition, ACE-informed social work practice should be sought.

 Second, depression had a partial mediating effect in the relationship between ACEs and suicidal ideation, such that ACEs and depression were significantly positively correlated, and depression and suicidal ideation were positively correlated. This implies that ACEs directly influence suicidal ideation but may also lead to an increase in suicidal ideation via the depression pathway. 

 Third, social ecological resilience moderated the mediation effect of depression between ACEs and suicidal ideation. At all levels of social ecological resilience (high, mean, and low), the moderation effect was significant. The results provide practical implications that, in turn, can be used to prevent or mitigate suicidal ideation in early adulthood by strengthening social ecological resilience. Thus, it would be useful to apply the social ecological resilience practice model to intervene the influence of ACEs on suicidal ideation in early adulthood.

 Despite these efforts, this study was limited by the inability to consider individual and special circumstances in addition to risk and protective factors identified based on previous studies. Therefore, a more in-depth qualitative analysis of suicidality in early adulthood will be helpful in preparing specific suicide prevention strategies in social work practice.
